Film Permit Application & Forms

Film permit applications will not be processed until all documents are received.

Forms must be faxed or emailed at least 48 hours prior to filming .

Required Forms

• Film Permit Application (pdf)
• Hold Harmless Agreement (pdf) 
• Business Tax Certificate (pdf) 
• Insurance Binder

Original copies of the Insurance Binder, Hold Harmless Certificate, Additional Insured Policy, and Application for Business Tax Certificate must be received at the address above within 10 days after the scheduled filming begins. Failure to comply will result in the closure of the production company account. No future film permits will be issued to that production company.
