Submit a Claim for Damages

Step 1.Fill out the Claim for Damages form

Please fill out a "Claim for Damages" form if you believe the City is liable for damage to your property or personal injury.

Step 2.Include your information and documentation with the Claim Form.

Please include with your claim form any documents that may be relevant to the incident, i.e., receipts, photos, and medical records. This is optional; however, if the City does not have enough information to evaluate your claim, it may be rejected as “insufficient” and you may be required to submit additional information.

Step 3.Submit your claim

Note: Approval or denial of your claim may take up to several weeks or months, depending on the nature of the claim.
